hammer2 - Add media dump command, improve help output

* Add 'hammer2 show <device>'.  This will dump the contents of the
  hammer2 filesystem structure on-media.

* Improve usage() output.

* Rename 'hammer2 debug <host>' to 'hammer2 shell <host>'.
