Mop up remains of the ibcs2/streams/svr4 removal:

* Remove streams(4) and svr4(4) manual pages.

* Add associated modules and their manual pages to the list of files
  to be removed upon 'make upgrade'.

* Remove IBCS2 and SPX_HACK options.

* Change M_ZOMBIE definition back to static.

* Fix miscellaneous references & comments.
