Clean up the manual pages of mount_hammer(8) and newfs_hammer(8) a bit.

* In HISTORY, always refer to the (odd) development version numbers,
  not the (even) release ones.

* Start sentences on a new line, add some markup, comment out empty
  sections.

* Sort options.

* Fix typos.
