sound/pcm: fix calculation of PCM channel timeout

this fixes a "pcm0:virtual:0:dsp0.2: play interrupt timeout, channel dead" error
and sound dying after a few seconds of playback.
I encountered on my Thinkpad X40. It was discussed on freebsd-current in 2004[1] but that code is no longer present in FBSD while we still have it.

The patch removes a line fixing a channel timeout to 1 tick instead of the value calculated immediately beforehand, thus restoring the calculation.

The support for Position Independent Executables came from FreeBSD.
Unfortunately, a bug in their initial commit was also carried over.
Removing the GCRT define from the i386 crtstuff makefile will stop
PIE binaries from generating *.gmon files as they are executed.  The main
software that suffers this are cups and samba.  Rebuilding those ports
after rebuilding world will fix this issue on the i386 platform, the only
one affected.

kernel/ddb: Fix examine's z format (for signed hex).

This was previously attempted to be printed with a special %z conversion
specifier of kprintf(). But our kprintf() doesn't support this specifier.
Either support for it got lost along the way or we never had it, I didn't
check.

Since we want to get rid of non-standard conversion specifiers (because
they require compiler modification for silencing format warnings), use a
db_format_hex() function that does the same as %z was supposed to do.

Crypto volumes marked as "early" in crypttab(5) will be activated
before mounting file systems marked as "early" in fstab(5) are
mounted read/only. Finally the rest of the crypto volumes is set up.

The early-mounted filesystems are re-mounted read/write along with the
other file systems.

This allows to place key files for all but one filesystem on one that
is accessed first, thus avoiding repeated entry of passphrases while
maintaining security.

Instead, align it ourselves and keep the original address for kfree().
This is a bit hacky but not too hacky (and it is done the same way in
padlock(4)).

In the middle run, we probably need some kmalloc() flavor that can do
alignment.

however, AcpiSetupGpeForWake() will disable GPE, which causes devices
like button and lid to stop function if they have _PRW object.  So for

* Fix an apparent timing race where a USB command does not complete
  prior to the poll issued with the command.  Perform another poll
  in a loop to deal with it.

* Problem appears to occur w/ohci's early poll prior to ehci attachment.

systimer: Fix statclock() intr% counting for i8254 interrupt cputimer

If i8254 is used as interrupt cputimer, it will send IPIs to APs to get
statclock() called.  In order to detect fast interrupt, statclock()
tests gd_intr_nesting_level to see whether it should bump intr% or not,
however, during IPI processing, gd_intr_nesting_level is bumped up, which
tricks statclock() to believe that intr% should be incremented.  This
usually leads to 100% interrupt time on APs reported by utilities like
systat(1).  To solve this problem:

- Pass in_ipi to systimer functions to indicate whether the functions are
  called during IPI processing or not.
- In statclock(): if it is called during IPI processing, using
  (gd_intr_nesting_level - 1) to test whether intr% should be incremented
  or not.

Additional cleanup:
- Make systimer_init_*() interface stricter, by using "systimer_func_t"
  instead of "void *"
